2016 P3R Sand Registration
 

Registration is now open for 2016 for P3R Sand. Please select the session below and complete all required information:

    To complete registration, please make sure the following items are completed and mailed with your registration confirmation page...
     
    1) KRVA/USAV Registration (Juniors Only): All P3R Sand members must have a current USAV registration to register. If you were a juniors player this past club season for P3R or for ANY other club in ANY region, your membership is already up-to-date and all you need to do is send us a copy of your USAV WebPoint Membership Card. Please also enter your USAV membership number below.
     
    If you were NOT a USAV juniors member this year, you must sign up for a FULL juniors membership. Go to the KRVA Registration Page to apply for registration. If you have NOT registered using USAV's WebPoint System before, click on "New Member" if your daughter has not been a registered member before or follow the instructions under "Region Changes" if you were previously a member in another region. If you were a KRVA member last, click on "Webpoint Login" to renew your membership. Select P3R as your club affiliation. USAV registration may be paid directly by credit card on the website. Once registration is complete, print your updated membership card and mail it in with your registration. The new membership number must be entered below. Click HERE to complete your USAV registration.
     
    2) P3R Sand Registration: All player registration will be submitted online, then by mail. Complete the form below and print the following confirmation page that appears after you submit the form, and mail it in with a check for $650 made out to "P3R Volleyball" and mailed to P3R Sand, P.O. Box 23183, Pittsburgh, PA 15222-6183
     
    3) Register with a Partner: Participants are encouraged to register with their doubles partner. Make sure they register as well and include their name in the space provided below.
     
    Space is limited so please register early.

    By submitting this registration I agree that, being a parent or legal guardian of the child, does hereby affirm that the applicant is in good health and suffers from no illness, disability, or condition that requires the taking of medication on a regular basis unless that condition is disclosed and approved. Furthermore, the undersigned has no knowledge of any reason the applicant cannot participate in rigorous physical activity. The undersigned hereby expressly agrees to be responsible for any medical bill incurred in the treatment of any illness or accident. In the event of any such accident or injury, I hereby consent to allowing any of the club coaches/staff to procure any medical treatment deemed advisable on behalf of my child or ward without prior consent.
     
    I understand that as a condition of admittance to the program, the undersigned of behalf of all parents and guardians, and on behalf of the applicant, hereby releases P3R Volleyball Club and its staff from any and all liability from injury or illness, mental or physical, suffered by the player during or related to any club activity unless caused by willful or gross negligence by the person or entity against whom the claim is made.
